Overview
This innovative intervention is expected to promote the adoption of changes in practice systems by enhancing the capabilities and self-efficacy of clinicians to address the primary domains of cancer survivorship quality. Practice system changes will be implemented to identify cancer survivors receiving general medical care in the practices. Learners will obtain or develop treatment summaries and survivorship care plans for those patients based on best-evidence and guidelines.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Expand clinical and non-clinical supportive resources.
  2. Self-report an increase in knowledge regarding the phases, current trends, models of care, and evidence-based practices of cancer survivorship in order to reduce the long-term sequelae of cancer treatments.
  3. Improve quality of life for those diagnosed with cancer through further enhancing or providing educational resources to healthcare providers.
  4. Increased awareness of the options in both clinical and psychosocial treatment of cancer patients.
  5. Identify current trends in oncology intended to reduce long-term sequelae of cancer treatment.
