Overview
This activity will provide an overview of advanced care planning (ACP), types of ACP, quality of documentation, and common barriers to ACP. The activity will also highlight the importance of shared decision making and understanding when to initiate discussions about ACP, the role of oncology nurses and the impact of ACP on patient outcomes.

ANCC Learning Outcomes
At the completion of this activity, learners will:

  1. At the completion of this activity, the earners will articulate increased knowledge of advanced care planning and its impact on patient outcomes.

REQUIREMENTS FOR SUCCESSFUL COMPLETION

To receive NCPD hour for this activity, the participant must:

  • Create an account in the Professional Education Portal (PEP) and Register for this activity.
  • Pair Mobile Number to your PEP Account, then TEXT your attendance to 1-844-912-1333 up to 2hrs after the course has ended using the Activity ID provided by the Facilitator during program.
  • Complete a participant evaluation in PEP, under the MyCE tab, within 7 consecutive days of the learning activity where you will also have immediate access to your Certificate.
