As the number of cancer survivors increases, there is a parallel increase in the delivery of coordinated evidence-based care according to evolving discoveries in survivorship science. Researchers, clinicians, policy-makers, and patients often do not recognize or distinguish the specific skills and knowledge required to manage long-term follow-up of cancer survivors. Recent advancements in cancer survivorship research are under-recognized and under-utilized by these groups. This series will address this practice gap by addressing strategies to ensure that survivorship care plans are routinely provided to cancer survivors, adhered to and integrated into the survivor's long-term care by the healthcare team. 

1. Apply the benefits to cancer survivors and their families by using an interprofessional approach to survivorship research and care.
2. Integrate research that will encourage healthy lifestyles, wellness, and prevention in the growing population of cancer survivors.
3. Utilize cancer survivorship research to address the unmet needs of long-term care of survivors.
